LTGR is working with recovery organizations, treatment providers, and community organizations to create clear, direct routes into healthcare for the people they serve.

The Recovery Health Partnership Initiative brings LTGR's clinical care into formal partnerships with organizations already working in recovery communities.

Together, we establish how people connect to care, what happens after that connection is made, and how LTGR carries the healthcare process forward.

Addressing a curable condition.

Hep-C is one place where this work is being put into practice.

Through Hepatitis C Free Recovery Communities™, LTGR is creating direct routes from recovery organizations into Hep-C testing and treatment.

That may mean LTGR testing onsite, accepting direct referrals, or providing a dedicated intake route that allows someone to connect with us directly.

LTGR then manages the Hep-C care process through confirmed cure.

Built on experience.

LTGR has treated Hepatitis C for years. More than 3,000 people have reached cure through our collaborative care.

In 2024 alone, we treated approximately 90 people for Hep-C, including patients who received care through our free Hep-C program.

Many continued or returned for healthcare after treatment. Among approximately 40 insured patients, roughly half went on to establish primary care with LTGR.

That relationship created access to primary care and, when needed, HIV prevention, behavioral health, chronic disease management, preventive care, and other healthcare services.
