Your first visit is in person. After that, televisits are available when it makes sense. A patient portal when it helps, a real person when it matters.

Reach out
Call us or fill out the new patient form. We'll help you figure out the next step. STI testing is available on a walk-in basis.

First visit
In person, at the clinic. Plan on about an hour. We'll go through your history and what you came in for. You do most of the talking.

Your plan
You leave with a plan. Tests, prescriptions, referrals, next visit. Nothing left vague.

We partner with you
Labs, referrals, prescriptions, and follow-up are part of the plan. Results and next steps are shared with you along the way.

Between visits
Reach us through the patient portal or by phone. If something feels urgent, call the clinic and we'll help you figure out the next step.
Bring
- Insurance card
- Photo ID
- Current medications
- Recent labs, if you have them
If you have none of these, come anyway.

LTGR accepts Medicare, Medicaid, and many commercial insurance plans. Coverage varies by plan and network. Contact us or your insurer to confirm participation before your visit.
If cost is a worry
Sliding fee support is available. See the section below.
Cost should not keep you from care.
LTGR offers sliding fee support based on financial need. Depending on eligibility, this may reduce the cost of care or cover it entirely.
You can download the application and required paperwork before your visit, or ask us for help completing it when you come in.
We won't pretend barriers don't exist. We'll help you find a way through them.

Do you have a patient portal?
Yes. You can message the clinic, see results, request refills, and manage appointments through the portal. If you'd rather call, that works too.
Can I be seen by televisit?
Your first visit must be in person at the clinic. After that, many follow-up visits can be done by video when it's the right fit. We'll help you decide what makes sense.

What if I'm worried about cost?

Do you see minors?
We see patients 18 and older. For younger patients we can help point you to a pediatric provider.
Is what I tell you private?
Your health information is protected under HIPAA and handled according to our Notice of Privacy Practices.
