PRIMARY CARE

Your health doesn't happen one thing at a time.

Blood pressure affects more than blood pressure. Diabetes changes heart and kidney risk. Sleep can affect mood, energy, weight, and concentration. Medications for one condition can matter when treating another. Primary care is where those pieces begin to make sense together.

The more we know about you, the more useful that care becomes.

Some things need more than one visit.

Diabetes. High blood pressure. High cholesterol. Thyroid disease. Asthma. Heart disease. Conditions that change over time need care that changes with them.

That means watching the numbers, but also paying attention to how treatment is working in your actual life.

A medication may need adjusting. A new symptom may change the plan. Something that looked insignificant six months ago may mean more now.

We keep looking.

Diabetes, heart health, and everything between them.

Diabetes is more than blood sugar. Blood pressure, cholesterol, kidney function, medications, eye care, foot health, nutrition, activity, and cardiovascular risk all become part of taking good care of it.

Heart health works much the same way. Blood pressure, cholesterol, diabetes, smoking, family history, kidney health, age, and other factors build a picture over time.

That gives us more ways to prevent problems before they become bigger ones.

Knowing you matters.

A blood pressure reading means more when we know what yours usually looks like. A new symptom means more when we know what has changed.

A medication problem is easier to solve when you can tell us what you are actually taking, what you stopped taking, what it costs, what makes you feel terrible, and what simply does not work in your life.

That kind of familiarity takes time. It is also one of the reasons primary care matters.

A lot can stay right here.

Checkups, sick visits, chronic disease management, medications, preventive care, sexual health, hormone therapy, and many of the things that come up between them can be handled here.

Our lab is here too, so bloodwork usually does not require another stop.

When a specialist can add something we need, we make the referral. The rest of your care stays connected.

Prevention changes as we do.

What needs attention at 30 may be different at 45, 60, or 75.

Screenings, vaccines, blood pressure, cholesterol, diabetes risk, cancer prevention, bone health, sexual health, and family history all matter at different times and in different ways.

Primary care gives us a place to keep track of that over time instead of starting over with every visit.

You can start with whatever brought you in.

A physical. A medication question. Diabetes that has been difficult to control. Blood pressure that keeps creeping up. Something new that does not feel right. Or simply realizing it has been too long since anyone looked at the whole picture.

Come in with that. We can start there.

We're here to help.

We accept Medicare, Medicaid, commercial insurance, and self-pay, with a sliding fee scale available for those who qualify.

If you are looking for a primary care provider, or simply need somewhere to begin, call us.

Same-day appointments are often available for establishing care. Follow-up for ongoing conditions is scheduled based on what you need.
